fRESH rX pROVIDER rEFERRAL

FreshRx for MOMs is a collaboration of Lazy Eight Stock Farm and Community Farm Alliance, funded by a USDA/NIFA GusNIP grant. All personal information is confidential. Aggregated data (anonymized, with no individual names or identifying information attached) will be used for reporting and evaluation and potentially shared publicly for educational or fundraising purposes.

Eligibility Criteria

FreshRx for MOMs participants must be:                                                              

• Currently pregnant(before 32 weeks)
• Currently enrolled in Medicaid
• Attending regular visits with a physician or midwife(telemedicine visits acceptable)
• The patient has given permission for a referral to Fresh Rx with Lazy Eight Stock Farm

HOW TO MAKE A REFERRAL:

Simply send an email to Carla Baumann, RN. You may also TEXT or CALL her at 859-200-4831. Be sure to include your contact information and agency/office name in addition to the participants name, phone number and email.
